Article 124 (1) Chief justice is always consulted in appointment of a Judge other than the Chief Justice. So, statement 1 is correct.
Article 124 (4) – A Judge of the Supreme Court can be removed by an order of the President, which is passed after an address by each House of Parliament supported by a majority and presented to the President in the same session and on the ground of proved misbehaviour or incapacity. So, statement 2 in incorrect.
Article 121 (3) (d) (i) – the salaries, allowances and pensions of Judges of the Supreme Court are charged on Consolidated fund of India. So, statement 3 correct.
In constitution, there is no mention of appointment of officers and staffs of the Supreme Court only after consultation with Chief Justice of India. In Fact Article 50 says that judiciary must be kept separate from executive. SO, statement 4 is wrong.
